Grade 0 Normal cartilage
Grade 1 Nearly normal (superficial lesions)
Grade 2 Abnormal (lesions extend < 50% of cartilage depth)
Grade 3 Severely abnormal (>50% of cartilage depth)
Grade 4 Severely abnormal (through the subchondral bone)

Lesion < 1 cm2 = microfracture / bone marrow stimulation technique +/- HA (Hyaluronic Acid Gel) / PRP (Platelet-Rich-Plasma)
Lesion 1-4 cm2 = Autograft OATS/mosaicplasty or MACI (matrix derived autologous chondrocyte implantation)
Lesion > 4cm2 = MACI or fresh osteochondral allograft or harvest from both knees autograft OATS
