Dr Nguyen’s principles for repair of knee cartilage lesion

Surgical Repair of Articular Cartilage Defects of the #Knee


ICRS (International Cartilage Repair Society) Grading System of Defects

Grade 0 Normal cartilage

Grade 1 Nearly normal (superficial lesions)

Grade 2  Abnormal (lesions extend < 50% of cartilage depth)

Grade 3  Severely abnormal (>50% of cartilage depth)

Grade 4  Severely abnormal (through the subchondral bone)


Dr. Nguyen’s principles for repair / reconstruction of cartilage lesions 


Lesion < 1 cm2 = microfracture / bone marrow stimulation technique +/- HA (Hyaluronic Acid Gel) / PRP (Platelet-Rich-Plasma)


Lesion 1-4 cm2 = Autograft OATS/mosaicplasty or MACI (matrix derived autologous chondrocyte implantation)


Lesion > 4cm2 = MACI or fresh osteochondral allograft or harvest from both knees autograft OATS

4.5 mm

6, 8, 10, 12 mm cylinder options 

Sizers/tamps

Donor 15 mm depth

Recipient 13 mm depth

90 deg angle to articulate surface

Rotate 90 deg clockwise then 90 deg counterclockwise 

Turn white knob to deliver first part of plug into recipient site followed by gentle taps

Finish with gentle taps on sizer
